By signing this form, covering any and all classes (both in-person & digitally/on-line) given by Culinary Kids Academy/Culinary Kidz LLC, I hereby indicate my understanding and agreement to the following; That my contact information may be utilized so I can be informed of future fun workshop opportunities (CKA will never share email information with third parties). I further understand that there are natural and inherent risks associated with participating in any cooking activities, including, but not limited to, the risk of physical injury or harm from knives, stoves, ovens and other kitchen equipment, hot water/oil, and food borne illness. I acknowledge and assume any and all risks associated with the culinary activity in which me, my family, and any other participants in my home/under my care will be participating and any damage, illness, and/or injuries resulting from such risks. I further agree to defend, indemnify, save and hold harmless Culinary Kids Academy/Culinary Kidz LLC, its members, owners, shareholders, directors, officers, managers, employees, and agents from any and all claims, actions, or causes of action, demands, damages, or liabilities arising out of or related to my participation in culinary classes. This waiver specifically includes any claims made by my family, other participants in my home and me arising from the alleged negligent acts of Culinary Kids Academy/Culinary Kidz LLC, its members, owners, shareholders, directors, officers, managers, employees, and agents. This waiver shall remain in force and effect and apply to all classes taken by me, my family or other participants in my home until revoked in writing by me and delivered to Culinary Kids Academy/Culinary Kidz LLC.

As a rule, Culinary Kids Academy/Culinary Kidz LLC does not use nuts or shellfish in its recipes. Culinary Kids Academy/Culinary Kidz LLC does, however, purchase ingredients that may contain the following manufacturer’s notice/disclaimer (or the like): “Product may be manufactured on equipment also used for nuts/shellfish/other allergens.” Thus, while Culinary Kids Academy/Culinary Kidz LLC aims to avoid cross-contamination, it cannot guarantee the total absence of cross-contamination. Participants with food allergies must be aware of this risk. Culinary Kids Academy/Culinary Kidz LLC does not assume any liability for, and I agree to hold Culinary Kids Academy/Culinary Kidz LLC harmless from adverse reactions from foods consumed and itemized food sources that my family, any other participants in my home and I may come into contact with during class and/or recipe preparation.

 

Lastly, I hereby grant and assign Culinary Kids Academy/Culinary Kidz LLC the irrevocable and unrestricted right to use and publish for promotional, trade, and advertising purposes in any manner or medium, including website and internet promotion, all photographic, video and digital images of me. Such photographic, video, and digital images will NOT include any personally identifying information. I hereby release Culinary Kids Academy/Culinary Kidz LLC from any and all royalties, claims, and liabilities relating to said photographs, videos, and digital images.
